All parents of students that will going on the Disney spring trip (May 3 - 6, 2012) need to be in attendance at the Disney Trip meeting on Thursday, March 1st at 7 p.m. (Forms due between 6:30 - 7 p.m.)
Turn in all paperwork (some notarized) to the chaperones at the table in the foyer of the theater entrance between 6:30 - 7 p.m. All papers must be fully completed and we will also need a copy of the front and back side of the student's insurance card. At the top of the copied ins. card page please print your child's name in the upper right hand corner.
If you do not have medical insurance please see the DMS secretary to purchase school insurance prior to the trip. The secretary, Rita Janik can also witness you sign the medical form and notarize your signature/date. You must sign it in front of her and not before you bring it to school. This has to be done before school only. Otherwise you can get it notarized at your bank of place of employment.
All packets will be sent home in the next few weeks with your child. Please complete everything and bring it to the meeting with you. Some sheets are on colored paper. You will also need to send in your 3rd payment of $150 for Feb 3rd. The final payment will be calculated for fundraiser credits and requested at the March 1st meeting. If no fundraiser credits the final payment will be $100.
